Clinical FAQs
Why would a clinician recommend cut-to-fit over pre-cut?
Cut-to-fit barriers are standard for new ostomates whose stoma is still edematous and changing size post-operatively — typically the first 6–8 weeks. They are also preferred for oval, irregular, or retracted stomas that don't fit a standard round pre-cut opening, and for patients managing peristomal skin complications where precise fit is critical to healing.
How do I cut the barrier correctly?
Measure your stoma using a stoma measuring guide at each change (especially in the early post-op period). Trace the correct size onto the white paper backing of the barrier, then cut with curved ostomy scissors for a clean edge. The opening should be within 1/8" (3mm) of the widest point of your stoma — snug but not pressing against the stoma tissue.
Which flange size should I choose — 40mm, 50mm, or 60mm?
The flange size determines what pouch you use, not how big your stoma is. Choose the smallest flange that gives you enough barrier surface area around your stoma for adhesion. Green (40mm) suits most average-sized stomas; Red (50mm) or Blue (60mm) are used when additional peristomal skin coverage is needed, such as with abdominal creases, larger body profiles, or wide peristomal skin irritation.
Can I use this barrier if my stoma is oval or irregular?
Yes — this is one of the primary clinical indications for cut-to-fit. Trace your stoma shape directly onto the barrier backing (using a template or tracing paper) and cut to that shape. This is not possible with pre-cut barriers, which are fixed round openings.
How long does this barrier typically last?
Standard wear Assura barriers are designed for 3–5 days under normal conditions. Wear time varies based on output type (liquid ileostomy output is more erosive than colostomy output), body perspiration, activity level, and peristomal skin condition. If the barrier is loosening before 3 days, consider the extended wear version or consult your WOC nurse.
Can the pouch be changed without removing the barrier?
Yes. The mechanical coupling allows independent pouch removal and replacement while the barrier stays in place. This reduces the number of barrier changes per week, minimizing adhesive trauma to the peristomal skin — an important consideration for skin integrity.
What accessories pair with this barrier?
For cut-to-fit barriers, Coloplast ostomy scissors are strongly recommended for clean, precise cuts. A stoma measuring guide ensures accurate sizing at each change. Brava barrier rings or paste can be used to fill any gap between the cut opening and the stoma for extra leak protection.
