What is bladder irrigation and when is it indicated?

Bladder irrigation is the instillation of sterile fluid into the bladder through an indwelling catheter — typically a Foley catheter — followed by drainage of the instilled fluid and any material it has mobilized. The purpose is to flush blood clots, sediment, mucus, or tissue debris that would otherwise block the catheter lumen or accumulate in the bladder, potentially causing obstruction, infection, or discomfort. The most common indications in clinical settings are post-operative bladder care following transurethral resection of the prostate (TURP), cystoscopy, or other bladder procedures; management of gross hematuria (blood in the urine) with clot formation; relief of catheter blockage from debris or sediment; and instillation of therapeutic solutions (such as antiseptic agents) under direct order from a clinician. Bladder irrigation is a clinician-directed procedure — it should not be performed without a clinical indication and should follow facility protocols for aseptic technique.

How is the bulb syringe used for bladder irrigation?

The bulb syringe is the delivery and aspiration tool for manual intermittent bladder irrigation. To use: draw up the prescribed irrigation solution into the 60cc bulb by submerging the tip in the solution and releasing the compressed bulb, allowing it to expand and fill. Clamp the catheter drainage tubing to prevent solution from flowing directly into the drainage bag. Disconnect or access the catheter irrigation port using aseptic technique. Insert the syringe tip into the catheter inlet port and gently depress the bulb to instill the 60cc of solution into the bladder. Allow the solution to dwell briefly if directed, or immediately release the clamp and allow the solution to drain passively by gravity into the collection basin. Repeat the cycle as directed by the clinical order. The tip protector should remain on the syringe tip during all handling until immediately before connection to the catheter port.
