How It Works — Donut vs. Ring Pessary
The Donut Pessary is essentially a thicker, more volumetric version of the ring pessary — and that distinction is what makes it clinically valuable for advanced prolapse. Where a ring pessary rests relatively flat against the vaginal walls, the donut's solid torus body fills the vaginal vault more completely, providing greater structural contact and support across a larger area. This makes it particularly effective when a vaginal vault has enlarged or lost elasticity due to aging or post-menopausal tissue changes, and when a ring pessary has slipped, been expelled, or failed to adequately support the prolapsed structures. The soft silicone construction allows the donut to be compressed somewhat during insertion to ease passage through the introitus — adequate lubrication is essential. Once in place, the pessary fits either along the length of the vagina like a ring, or snugly into the circumference of the vaginal apex where intrinsic support is present.
Removal requires lubricating the introitus, inserting an examination finger into the center of the donut, and pulling gently downward at a diagonal angle. Some patients can be taught to perform this independently, though the donut typically requires more clinical involvement than a ring pessary. Because of the greater vaginal wall contact, patients should be aware that more complete contact with the vaginal epithelium may increase the risk of discharge accumulation or mechanical tissue irritation with longer wear intervals — regular clinical follow-up is important.
