What is the difference between a one-piece and a two-piece ostomy system?

In a one-piece system, the skin barrier (wafer) and pouch are permanently bonded together as a single unit — the entire appliance is applied to and removed from the skin as one piece. In a two-piece system, the barrier and pouch are separate components that connect via a coupling ring; the pouch can be changed without removing the barrier. The one-piece system offers simplicity — fewer components, no coupling mechanism, and a lower profile against the body. The two-piece system offers flexibility — the barrier stays on skin while only the pouch is changed, reducing adhesive trauma for users who change frequently. The right choice depends on output type, change frequency, and personal preference.

What is Stomahesive and why is it used as the skin barrier?

Stomahesive is ConvaTec's proprietary hydrocolloid skin barrier material. It absorbs moisture from the peristomal skin while maintaining a secure adhesive seal, protecting the skin from irritation caused by stoma output. Stomahesive is specifically formulated for semi-formed to formed output — the type associated with most colostomies. For patients with liquid ileostomy output, a more erosion-resistant extended wear material (such as Durahesive) is typically indicated instead.

What does the transparent pouch allow the user to see?

The transparent film of the ActiveLife pouch allows the user or caregiver to visually inspect the stoma and monitor output without removing the appliance. This is clinically valuable during the early post-operative period when stoma color, size, and output characteristics are being monitored closely. Transparent pouches are also preferred by users who want to confirm stoma positioning during application. Some users later transition to opaque pouches for additional discretion once they are comfortable with their stoma.

How does the built-in charcoal filter work and when should the filter cover be used?

The charcoal filter uses activated carbon to adsorb odorous gas compounds as they pass through the filter membrane — gas exits the pouch without detectable odor. The filter is passive and requires no action from the user. A waterproof filter cover sticker is included and should be applied before bathing, swimming, or prolonged water exposure to prevent the charcoal from becoming saturated with water, which temporarily reduces filter effectiveness. Remove the cover after water activities to restore normal gas venting.

When is a pre-cut opening clinically appropriate vs. cut-to-fit?

Pre-cut openings are appropriate when the stoma has reached a stable, consistent size — typically 6–8 weeks or more after surgery, once post-operative edema has resolved. The stoma must be round and match one of the available pre-cut sizes (1" or 2"). If the stoma is oval, irregular, or a size between the available pre-cuts, a cut-to-fit barrier is more appropriate. Using a pre-cut opening significantly reduces preparation time at each change and eliminates the risk of an incorrectly trimmed opening.

Why is this appliance appropriate for semi-formed to formed output only?

Stomahesive is formulated for the moisture levels associated with semi-formed to formed colostomy output. Liquid output — such as ileostomy effluent — is enzymatically aggressive and erodes Stomahesive faster than it is designed to withstand, reducing wear time and increasing leakage risk and peristomal skin breakdown. Patients with liquid output should use an extended wear barrier (Durahesive-based products) rather than Stomahesive. The ActiveLife system is not recommended for ileostomy patients.

Is the one-piece system suitable for patients with limited dexterity?

Yes — the one-piece system can be easier for patients with limited hand strength or dexterity because there is no coupling ring to align and click into place. The entire appliance is pressed directly onto the skin in one step. The flexible, lightweight construction also makes it easier to conform the barrier to the body without applying significant pressure. However, patients who need to change frequently may find the two-piece system more skin-friendly since it avoids repeated adhesive removal.
