Molnlycke Mepilex Border Sacrum Dressing — Deep Defense Technology, Safetac Silicone, 2 Sizes
The only five-layer sacral foam dressing clinically proven to reduce pressure ulcer risk by up to 88%. Mepilex Border Sacrum is Molnlycke's purpose-built sacral dressing, engineered with proprietary Deep Defense® Technology to protect high-risk tissue against the combined forces of pressure, shear, friction, and microclimate — the four primary mechanical drivers of sacral pressure injury. Its Safetac® silicone wound contact layer bonds gently to periwound skin without adhering to the wound bed, sealing wound margins to prevent exudate leakage and maceration while allowing pain-free, atraumatic dressing changes. Shaped specifically for the sacral anatomy with application tabs and an improved gluteal seal for secure, all-day conforming coverage. Available in two sizes.
✔ Deep Defense® Technology — Protects Against Pressure, Shear, Friction & Microclimate ✔ Safetac® Silicone — Pain-Free Changes, No Wound Disruption ✔ Clinically Proven — Up to 88% Reduction in Pressure Ulcer Risk ✔ Sacral Shape with Gluteal Seal — Stays in Place, Conforms to Anatomy

Indicated For — Uses
- Prophylactic pressure ulcer prevention in high-risk patients — hospital, long-term care, and home settings
- Management of existing sacral pressure injuries — all stages with medium to high exudate
- Patients at elevated risk due to immobility, incontinence, or compromised skin integrity
- Wounds requiring protection from shear and friction forces during repositioning
- Post-surgical or post-procedural sacral wound coverage
- Any sacral or perianal wound requiring a moist wound environment with secure, conforming coverage
How Mepilex Border Sacrum Works — The Five Layers
Sacral pressure injuries are among the most difficult wounds to prevent and manage because the area is constantly subjected to multiple simultaneous forces — downward pressure from body weight, shear from repositioning, friction from linen and incontinence products, and a persistently warm, moist microclimate. A standard foam dressing addresses only exudate absorption. Mepilex Border Sacrum's five-layer architecture is engineered to address all four mechanical risk factors simultaneously while managing moisture and protecting skin integrity throughout the wear period.
- Layer 1 — Safetac® Silicone Wound Contact Layer: Soft silicone adhesive that bonds to intact periwound skin but not to the wound bed or fragile granulating tissue. Seals wound margins to block exudate leakage onto surrounding skin. Allows dressing removal without stripping skin, tearing tissue, or disrupting the wound — the core mechanism for pain-free dressing changes
- Layer 2 — Non-Woven Spreading Layer: Rapidly wicks and spreads exudate across the full absorbent core area to maximize fluid distribution and prevent pooling or strikethrough at any single point
- Layer 3 — Absorbent Foam Core: High-capacity polyurethane foam that absorbs and holds exudate away from the wound bed and periwound skin, preventing maceration while maintaining a moist wound interface
- Layer 4 — Super Absorbent Fiber Layer: Locks absorbed exudate in place under compression and repositioning — preventing fluid from being squeezed back toward the wound (rewetting) when the patient is moved or repositioned
- Layer 5 — Waterproof Film Backing: Bacteria-barrier outer film that keeps the dressing interior sterile, blocks external contamination, and allows limited moisture vapor transmission to manage microclimate — the film keeps the charcoal layer dry and maintains the dressing's structural integrity through the full wear period
Together, these five layers deliver the Deep Defense® Technology combination — the only sacral dressing construction with this specific architecture and the body of peer-reviewed evidence supporting its effectiveness in pressure ulcer prevention.
Deep Defense® Technology — What the Evidence Shows
- Clinically proven to reduce pressure ulcer incidence by up to 88% when used as part of a comprehensive pressure ulcer prevention protocol
- Supported by more than 80 published peer-reviewed evidence pieces — more than any other sacral foam dressing on the market
- Real-world health economic data from a cohort of over 1.03 million patients demonstrated an average reduction of $77 in per-patient treatment costs
- Protects against all four extrinsic risk factors: pressure, shear, friction, and microclimate management
- Improved gluteal seal design maintains secure adhesion at the inferior border — the area most vulnerable to lifting during patient positioning and incontinence care
Safetac® Technology — Why It Matters for Dressing Changes
In sacral wound care, every dressing change carries risk. Traditional adhesive dressings strip the stratum corneum on removal — damaging fragile periwound skin, disrupting healing tissue, and causing significant patient discomfort. Over repeated changes on vulnerable patients, this cumulative trauma can worsen the wound environment and damage surrounding intact skin. Safetac silicone adhesive bonds securely to intact periwound skin to hold the dressing in place and seal wound margins, but releases cleanly and atraumatically on removal — leaving the wound and surrounding skin undisturbed. This makes Mepilex Border Sacrum well suited for use on patients with fragile, paper-thin, or incontinence-compromised periwound skin where adhesive trauma from standard dressings is a documented clinical concern.
Application Guide
- Cleanse the wound and dry the periwound skin thoroughly before application
- Select a size with sufficient coverage to extend well beyond the wound margins onto intact skin — the silicone border must contact intact skin on all sides to seal
- Position the patient for optimal access — prone or side-lying is preferred for accurate placement
- Use the application tabs to fold and position the dressing accurately before pressing down — tabs allow repositioning before final adhesion
- Press the silicone border firmly against the periwound skin on all sides, paying particular attention to the gluteal seal at the inferior border
- For dressing removal: lift from a corner and peel back slowly and gently — Safetac technology allows removal without trauma or disruption
- Change when exudate approaches the dressing edge, or per clinical protocol — may remain in place for several days on non-infected wounds
Choosing the Right Size
- 6.3" x 7.9" (282055) — Smaller sacral coverage — best for smaller sacral wounds, targeted pressure ulcer prophylaxis in smaller patients, or focal sacral injury management where the larger size would impede positioning or incontinence management
- 8.7" x 9.8" (282455) — Full sacral coverage — the standard clinical choice for most adult patients; provides wide anatomical coverage across the full sacrococcygeal area and gluteal cleft, and is the size used in the majority of clinical studies supporting the Deep Defense evidence base
