Description

3M Promogran Prisma Rope — ORC + Collagen + Silver for Deep Tunneling Wounds, Biodegradable

The same clinically proven ORC + collagen + silver technology as Promogran Prisma Matrix — in a rope format that delivers it deep into tunneling wounds, sinus tracts, and undermined cavities where a flat dressing cannot reach. Promogran Prisma Rope is a sterile rope configuration of the Promogran Prisma Matrix composite — the same 55% collagen, 44% ORC, and 1% silver-ORC formulation that has demonstrated 3.4 times greater odds of wound closure in meta-analysis, now in a form that can be loosely packed into deep wound tracts where the ORC component inactivates elevated MMPs at the wound base, collagen provides scaffolding for granulation tissue from the deepest point of the wound outward, and ionically bound silver delivers an antibacterial barrier across the full depth of the tract. On contact with wound exudate, the rope transforms into a soft, conformable, biodegradable gel that fills and adapts to the irregular geometry of the wound cavity — no gaps, no dead space. The biodegradable gel is progressively absorbed into the wound tissue, so residual dressing does not need to be removed at the next change.

✔ Rope Format — Packs Into Tunneling Wounds, Sinus Tracts & Undermined Cavities    ✔ Same ORC + Collagen + Silver as Promogran Prisma Flat Matrix    ✔ Inactivates MMPs at the Wound Base — Removes the Stall Mechanism From Depth    ✔ Biodegradable Gel — Fills the Cavity, No Removal Required at Dressing Change    ✔ Antibacterial Barrier — Silver-ORC at Full Wound Depth

Product Details

Manufacturer Solventum (Formerly 3M / Systagenix / KCI)
Item Number MA057
Format Rope — sterile, for packing tunneling wounds, sinus tracts, and undermined cavities
Composition 55% Collagen, 44% ORC, 1% Silver-ORC (25% w/w ionically bound silver) — same as Promogran Prisma flat matrix
ORC Technology Absorbs and inactivates MMPs and elastases at the wound base — removes the biochemical stall mechanism from deep wound areas
Silver Ionically bound silver-ORC — antibacterial barrier against common wound pathogens across full wound depth
Gel Formation Transforms into soft conformable biodegradable gel on contact with exudate — fills and adapts to irregular wound geometry
Biodegradable Yes — gel absorbs progressively into the wound, residual dressing does not need to be removed at changes
Change Frequency Every 24–72 hours depending on exudate level — daily for heavily exuding or infected wounds, up to 72 hours for moderate wounds
Secondary Dressing Required — semi-occlusive or non-occlusive secondary dressing fixed with non-irritating tape
MRI Safety MRI Unsafe — contains silver, remove before MRI examination
Sterility Sterile — 5 per box
Storage Below 25°C (77°F), away from direct light
HCPCS Code A6021

Indicated For — Wound Types

  • Tunneling wounds — the primary indication, wounds with narrow or deep tracts the flat matrix cannot access
  • Sinus tracts requiring collagen ORC silver delivery at wound depth
  • Undermined wounds — broad shallow undermining where a rope can be packed along the undermined margins
  • Deep cavity wounds — pressure injuries, diabetic foot ulcers, and post-surgical cavities with significant depth
  • Diabetic foot ulcers with tunneling or undermining
  • Venous leg ulcers with cavity components
  • Pressure injuries Stage 3–4 with depth
  • Post-surgical wounds healing by secondary intention with depth or tracking
  • Any wound where the flat Promogran Prisma Matrix cannot achieve full contact with the wound base

Contraindications: Not indicated for third-degree burns, wounds with necrotic tissue, or patients with known sensitivity to silver, ORC, or collagen. May be used on infected wounds only when proper medical treatment is addressing the underlying infection. MRI Unsafe — remove before MRI.

How It Works — ORC + Collagen + Silver at the Wound Base

Deep tunneling and cavity wounds present a specific clinical challenge that flat dressings cannot overcome — the wound base is inaccessible. Standard flat collagen dressings, foam dressings, and silver dressings all deliver their active components at the wound surface only. In tunneling wounds, the wound base — often the most active site of MMP overexpression, bacterial colonization, and stalled healing — receives no direct treatment from a surface-applied dressing. Promogran Prisma Rope solves this by delivering the complete Promogran Prisma ORC + collagen + silver composite directly to the wound base through the tunneling tract. Once packed loosely into the tract, the rope transforms on contact with wound exudate into a soft biodegradable gel that fills the irregular geometry of the tunnel — the ORC absorbs and inactivates MMPs throughout the full depth of the tract, the collagen provides scaffolding for granulation from the wound base outward, and the silver-ORC delivers its antibacterial barrier across the entire packed length. The biodegradable gel absorbs progressively into the wound tissue between dressing changes, so it does not need to be actively removed — simplifying the dressing change procedure for deep wound management.

  • Rope format delivers ORC + collagen + silver to wound base in tunneling wounds — not just the surface
  • ORC inactivates MMPs at depth — removes biochemical barrier to healing at the wound base
  • Collagen scaffolding supports granulation tissue from the deepest point of the wound outward
  • Silver-ORC provides antibacterial barrier across the full packed length of the tract
  • Transforms into biodegradable gel — fills tunnel geometry, no dead space between rope and wound wall
  • No removal required — gel absorbed progressively into wound tissue between changes
  • Part of the Promogran Matrix Family — same 3.4x wound closure meta-analysis evidence base
  • Supports biofilm management — ORC + silver combination addresses both the biochemical and microbial barriers in chronic tunneling wounds
  • Change frequency adapted to wound condition — daily for heavy exudate or infection, up to 72 hours for moderate wounds

Promogran Prisma Rope vs. Flat Matrix — Choosing the Right Format

Choose Prisma Rope (MA057) When: Choose Prisma Flat Matrix (MA028/MA019) When:
Wound has a tunneling tract or sinus that a flat pad cannot reach Wound surface is accessible and a flat pad achieves full wound bed contact
Wound has significant depth or undermining requiring packing Wound is shallow enough for full flat coverage
Stage 3–4 pressure injury with depth needing base-to-surface healing support Stage 1–2 pressure injury or surface wound
Diabetic foot ulcer or venous ulcer with tunneling or undercut margins Standard flat ulcer without depth or tracking

How to Apply — Step by Step

Step Action
1 Prepare wound per standard protocol — irrigate the tract with saline, debride accessible necrotic tissue
2 Cut the rope to the length of the wound tract if needed — do not overpack
3 If wound is dry — moisten the rope with sterile saline before packing to initiate gel formation
4 Loosely pack the rope into the wound tract — do not pack tightly, the gel needs room to expand and conform
5 Ensure a small tail of rope remains accessible at the wound opening — aids identification at next change
6 Cover with a semi-occlusive or non-occlusive secondary dressing — secure with non-irritating tape
7 At dressing change — residual gel in the wound does not need to be removed, apply fresh rope as directed
