About Two-Piece Ostomy Systems

A two-piece ostomy system consists of two components: a skin barrier wafer that adheres to the peristomal skin around the stoma, and a pouch that connects to the barrier via a plastic or silicone coupling ring (flange). The barrier stays on the skin for several days while the pouch can be removed and replaced — or swapped between a drainable and closed-end style — without disturbing the barrier or the skin beneath it. This design reduces the number of barrier changes, minimizing adhesive-related skin stripping and peristomal skin irritation over time.

Flat barriers suit a stoma that sits at skin level or protrudes above the skin surface on a smooth peristomal plane. Convex barriers are indicated when the stoma is flush with or recessed below the skin, or when there are creases, folds, or irregular contours around the stoma that prevent the flat barrier from sealing effectively. Convexity presses inward on the peristomal skin, encouraging the stoma to protrude into the pouch opening and improving the seal. Convex barriers should be used under clinical guidance — incorrect convexity can cause pressure injury to peristomal skin.
